How do I report a communicable disease?
For Class A Diseases: During business hours, please call (740) 342-5179, option 2. After business hours, please call (740) 684-0004 to reach the on-call nurse.
For all other reportable diseases: Fax lab report and/or completed Ohio Confidential Disease Report Form to the health department at (740) 342-5540 or call the health department at (740) 342-5179, option 2 to report the communicable disease.
How much is a birth certificate or death record?
Certified birth and death records are $25.00.
Emergency Preparedness
This program is funded through the Centers for Disease Control and Prevention Public Health Emergency Preparedness grant. It ensures the Perry County Health Department (PCHD) is prepared to respond to infectious diseases; natural disasters; biological, chemical, nuclear and radiological events.
Environmental Health
This program oversees home sewage treatment systems; private water systems; plumbing installations and alterations; solid waste licensing and inspections; food service licensing and inspections; campground licensing; swimming pool licensing; mosquito surveillance and control; animal bites/rabies concerns; public health nuisances; construction and demolition debris (C&DD) Community Cleanup and Education Programs.

This program provides birth certificates; death certificates; paternity affidavits; burial/cremation permits.
